Community Navigator

Position Reports To: Supervisor

Hours: Full-time, occasional evening and weekend work required


 

Job Summary:

The Community Navigator will provide a key role in facilitating culturally and linguistically appropriate connections to wrap-around services that effectively link individuals, families and businesses to resources and education. They will work with the Associate Director and Supervisor to meet the goals outlined in the Scope of Work, including all deliverables.

 

Responsibilities and Duties:

Community Navigator will:

• Assess client needs and identify relevant community resources.

• Utilize cultural competencies to establish trusting relationships.

• Research and identify potential contacts and partners.

• Advocate for clients and provide ongoing support throughout the referral and connection process.

• Comprehensively & accurately gather data and documentation needed per grant requirements.

• Conforms with and supports organization’s quality assurance and improvement guidelines.

• Supports and adheres to the values and mission statement established by the AVPH Board of Directors.

• Maintain daily work schedule: Keep current Outlook calendar showing appointments as set the last day of the

workweek for the following week. Update as changes occur.

• Attend and participate in meetings and trainings as required and as authorized.

• Perform other duties as assigned by leadership.


Experience and Requirements:

• Bachelor’s degree, preferred in Social Work, Psychology, Human Services, Public Health, or a related field or at experience in the last two years for related work.

• Bilingual preferred (Spanish).

• Experience in networking and relationship building.

• Experience working with culturally diverse communities with the ability to be culturally sensitive and appropriate

• Excellent written/oral communication skills.

• Demonstrated track record with respect to detail orientation and accuracy.

• Must demonstrate excellent skills in Microsoft excel, word, outlook and the ability to learn new data programs.

• Work a flexible dayshift schedule that includes some Saturdays, Sundays, and evenings.

• Current and maintained CA driver’s license.

• Current/maintained auto insurance.

• Own transportation for work-related purposes.

• Must pass background check and TB test clearance.

• Must be fully vaccinated for COVID-19.

• Knowledgeable about local community resources and able to refer families to appropriate agencies.

• Knowledge of departmental policies, procedures, and organizational relationships.

• Knowledge of office practices, processes, and computer software programs.

• Ability to function as a member of a multidisciplinary team.

• Ability to assess families across a broad spectrum of areas, recognizing professional boundaries and the need for

consultation from other disciplines.

• Ability to plan and effectively manage complex, long-term projects.

• Ability to work independently and with little to no supervision.

• Ability to meet schedules and deadlines of the work area.

• Ability to compose routine correspondence and reports.

• Ability to maintain the confidentiality of client and department information.

• Ability to adhere to safety rules and regulations.

• Ability to establish positive rapport with diverse populations, utilizing cross-cultural skills and knowledge of cultural and religious differences, attitudes, and practices.

• Ability to develop relationships with providers and community organizations and provide input on communication and outreach strategies.

• Skill in organizing and maintaining accurate records.

• Proficiency in Word, PowerPoint, Outlook, and Excel.

• Written and verbal communication and interpersonal skills preferred.

• Ability to multi-task and effectively prioritize.

 

Physical Requirements and Working Conditions:

• Primarily in off-site community centers, community events, office setting and/or via a virtual platform.

• Working with overburdened, often high-risk clients of low economic and varied cultural backgrounds, living in high[1]risk areas.

• Office, and possible community settings.

• Work is often performed under stressful and emotional conditions.

• This position requires lifting/carrying up to 25 lbs., stooping, bending, squatting, sitting on the floor, walking, running (may be needed).
