Job Title: Community Navigator Supervisor

Position Reports To: Associate Director

Hours: Full-time, occasional evening and weekend work required

 

Job Summary:

The Community Navigator Supervisor will be responsible for overseeing the implementation of the Community Navigator grant.  They will provide a key role in facilitating cultural and linguistically appropriate connections to wrap-around services that effectively link individuals, families and businesses to resources and education. The Supervisor is responsible for guiding and providing processes and procedures for the team.  The Supervisor will coordinate outreach events, lobbies and activities.   The Supervisor will work with the Associate Director to meet the goals outlined in the Scope of Work, including all deliverables.

 

Essential Duties and Responsibilities:

  • Support and trains staff on identifying clients’ strengths, needs, barriers, and risks. 
  • Develop relationships with providers, community organizations and small businesses.  
  • Explore and identifies new referrals and resources for community members, community organizations and small businesses.
  • Organize, register and attend community outreach events. 
  • Work closely with staff to track referrals and outreach efforts.
  • Provides one on one reflective supervision for individual staff and supervisory staff, when needed.
  • Focuses reflective support and exercises/training to prevent staff compassion fatigue; to enhance self-care practices and maintain a work-life balance.
  • Work closely with the Associate Director to implement documentation, training, procedures, policies, and protocols.
  • Required to drive to outreach sites weekly
  • Conduct ongoing team building activities to enhance employee relations and teamwork.
  • Mentors and assists with ongoing training of employees to ensure that they are modeling empathy and providing holistic and strength-based services.
  • Assist with, and provides, ongoing staff development related to the issues and needs of clients.
  • Lead and supervise a team, providing guidance, support, and mentorship to ensure high-quality service delivery.
  • Work with the Associate Director to assist with any staff trainings per grant guidelines, funder requirements, and position related certification needs.
  • Work closely with the Associate Director to ensure all program requirements are being met. 
  • Responsible for annual Performance Evaluations.
  • Participate in periodic grant-related workshops, webinars, conference calls and regional meetings.
  • Maintain daily work calendar and schedule.
  • Other duties as assigned.

Education, Experience & Requirements:

  • Bachelor’s degree preferred or 3 years of experience conducting and organizing outreach activities.
  • A solid understanding of, or relevant experience, in supervising diverse staff with humility, as well as providing support to staff in stressful work environments.
  • A willingness to engage in building reflective practice (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.)
  • Experience working with family services and programs which embrace the concepts of family centered and strength-based provision.
  • Experience working with culturally diverse communities and families with the ability to be culturally sensitive, respectful, and appropriate.
  • Experience working collaboratively as a member of a multi-disciplinary team.
  • Have a solid understanding of, and experience in managing staff or program projects.
  • Excellent written/oral communication skills.

Knowledge, Skills & Abilities:

  • Knowledge of community resources and how to access them.
  • Skill and ability to assess risk and respond accordingly using best practice within the law and adhering to the policies and procedure.
  • Strong verbal, grammatical and written skills.
  • Proficiency in Microsoft Office Software.
  • Ability to work independently and with little to no supervision.
  • Conforms with and supports organization’s quality assurance and improvement guidelines.
  • Supports and adheres to the values and mission statement established by the AVPH Board of Directors.
  • Ability to work collaboratively as a member of a multi-disciplinary team.
  • Ability to adhere to safety rules and regulations.
  • Ability to co-case manage, establish, and maintain personal/programmatic boundaries, while providing supportive services      
  • Ability to communicate and interview effectively to solicit thorough, pertinent and accurate information
  • Ability to relate to participants from a family focused and strength-based paradigm
  • Ability to engage families and sustain participant activity in services
  • Ability to multi-task, maintain and file records/reports/documents in an organized and timely manner
  • Demonstrate success in working as a member of a team and developing effective working relationships with staff, community-based organizations, county facilities and residents. 
  • Have a demonstrated track record with respect to detail, orientation, and accuracy.
  • Empathy, warmth, and the ability to establish trusting supportive relationships with an ethnically diverse population.
  • Skill in writing to articulate concepts and ideas in notes and reports.

Administrative:

  • Function as part of the interdisciplinary leadership team.
  • Attend meetings and trainings as required, locally, as well as outside of the AV region when necessary.
  • Perform all duties in a professional, positive, respectful, and caring manner.

 

Required Licensure and/or Certifications:

Current and maintained CA driver’s license

  • Current/maintained auto insurance
  • Own transportation for work-related purposes
  • Meet basic health requirements, background check, and physical job standards
  • Bi-lingual, English/Spanish, desirable
  • Vaccinated for COVID-19 recommended
  • TB test required

Working Conditions:

  • Work in flexible settings, such as: Hybrid, In-office/desk and/or Virtually; In participant homes/ residential locations; Community; and other business locations as needed, to perform job duties.
  • Work with low economic and varied cultural backgrounds, with individuals living in high-risk, etc. 
  • Able to maintain HIPAA requirements with confidential information, safely and discreetly, as required, when working in various settings. 
  • Able to follow all safety and communication protocols.
  • Work may require sitting for occasional extended lengths of time.
  • Work may require periods of case management and documentation hybrid utilizing electronics. 
  • This position requires lifting/carrying up to 25 lbs., reaching, stooping, bending, squatting, sitting on the floor, walking, running (may be needed).
